Are you tired of a typical Primary Care Practice? We are seeking a dedicated Nurse Practitioner to join our Primary Care Team. In this role, you will work closely with advanced practice clinicians in a team-based care model. The Advanced Practice Provider will collaborate with interdisciplinary healthcare teams to deliver high-quality, patient-centered care. Within the advanced practice practitioner-physician relationship, this provider has the ability to exercise autonomy in medical decision making and provide a broad range of diagnostic and therapeutic services. May include responsibility for administrative duties and includes additional duties and parameters of specific individual contract and/or continuing education requirements.

Requirements

3+ years of experience in outpatient Family Medicine is preferred.
Ideal candidate will be flexible and eager to work as a part of a team in a patient-centered medical practice
Current patient panel is Pediatrics to Geriatrics
Monday through Friday Clinic schedule
