We're a compassionate team of medical and social service professionals in a supportive, affirming environment. The three petals of the Trillium flower represent our comprehensive model of care - we treat the physical, emotional and social health of our patients.
We provide a full range of services at our downtown location in Rochester, NY, including Primary Care and a Pharmacy – all under one roof. We also offer several other programs and services, including Pathway Pediatrics, the Center for Gynecological Care & Wellness, the MOCHA Center and Harm Reduction Services.

Company History
We began as a small HIV/AIDS clinic in the 1980's during difficult times, and we’re proud of our legacy as the leading LGBTQ+ healthcare provider in Rochester.
Today, Trillium Health is a Community Health Center that provides extraordinary care for all, including LGBTQ+ health, ensuring equitable, judgement-free, and affordable care.
